What is an Insulin (Fasting) Test?
The Insulin (Fasting) test measures the amount of insulin in your blood after a period of fasting. Insulin is a critical hormone produced by the pancreas that acts as a key, allowing blood sugar (glucose) to enter your cells to be used for energy. This test is primarily used to evaluate metabolic health and determine if your body is using insulin efficiently — or if you are developing insulin resistance, a condition where the body requires higher-than-normal insulin levels to keep blood sugar stable.
Why Monitoring Fasting Insulin is Crucial
Standard glucose tests often only show a problem once diabetes has already developed. A fasting insulin test can identify metabolic risks much earlier:
- Detecting Insulin Resistance — Identifies if your pancreas is overworking to maintain normal sugar levels, a precursor to Type 2 Diabetes
- PCOS Management — High insulin is a major driver of Polycystic Ovary Syndrome, contributing to hormonal imbalances and weight gain
- Diagnosing Hypoglycemia — Helps determine if low blood sugar is caused by overproduction of insulin such as from an insulinoma
- Metabolic Syndrome Screening — Evaluates risks related to obesity, high blood pressure and cardiovascular health
Normal Fasting Insulin Range
| Fasting Insulin Level | Interpretation |
|---|---|
| 2 – 6 mIU/L | Optimal – excellent insulin sensitivity |
| 6 – 10 mIU/L | Normal – acceptable range |
| 10 – 20 mIU/L | Borderline – early insulin resistance possible |
| Above 20 mIU/L | High – significant insulin resistance likely |
| Below 2 mIU/L | Low – possible Type 1 diabetes or insulinoma |
Your doctor will interpret your fasting insulin result alongside your FBS, HbA1c and HOMA-IR calculation.
What is HOMA-IR and Why Does It Matter?
HOMA-IR = (Fasting Insulin x Fasting Glucose) ÷ 405
A HOMA-IR below 1.0 indicates excellent insulin sensitivity. Above 1.9 suggests early insulin resistance. Above 2.9 indicates significant insulin resistance. Your doctor will use this calculation if both fasting insulin and FBS are ordered together.
When is a Fasting Insulin Test Recommended?
Your doctor may recommend a Fasting Insulin test if you:
- Have been diagnosed with PCOS (Polycystic Ovary Syndrome)
- Are overweight or obese with difficulty losing weight
- Have borderline fasting blood sugar or prediabetes
- Experience frequent hypoglycemia (low blood sugar episodes)
- Have a strong family history of Type 2 diabetes
- Have high triglycerides or low HDL cholesterol
- Have unexplained fatigue, brain fog or sugar cravings
- Are being investigated for metabolic syndrome
- Are suspected of having an insulinoma (insulin-producing tumour)
What Conditions Does Fasting Insulin Help Diagnose?
- Insulin resistance
- Prediabetes and Type 2 Diabetes risk assessment
- Polycystic Ovary Syndrome (PCOS)
- Metabolic syndrome
- Hypoglycemia (low blood sugar)
- Insulinoma (insulin-secreting pancreatic tumour)
- Non-alcoholic fatty liver disease (NAFLD)
How is the Fasting Insulin Test Performed at Apollo Diagnostics?
Apollo Diagnostics uses Chemiluminescence Immunoassay (CLIA) technology for fasting insulin testing — a highly sensitive and specific automated method that delivers accurate hormonal measurements at very low concentrations. A single blood sample is collected after your fasting period and processed at our NABL-certified central laboratory.
