What is a Blood Group & Rh Typing Test?

The Blood Group and Rh Typing test is a fundamental diagnostic procedure used to determine an individual's ABO blood type (A, B, AB or O) and their Rh factor (Positive or Negative). This gives you your complete blood group — for example A+, O–, AB+ or B+. Understanding ABO Blood Groups

The ABO blood group system classifies blood into four types based on the presence or absence of A and B antigens on the surface of red blood cells and corresponding antibodies in the plasma:

Understanding the Rh Factor

The Rh factor (Rhesus factor) is determined by the presence or absence of the D antigen on the surface of red blood cells:

  • Rh Positive (Rh+) — the D antigen is present on red cells. About 93–95% of Indians are Rh positive. Rh+ individuals can safely receive both Rh+ and Rh– blood.
  • Rh Negative (Rh–) — the D antigen is absent. Only 5–7% of Indians are Rh negative. Rh– individuals must only receive Rh– blood to avoid a dangerous immune reaction called haemolytic transfusion reaction.

Why is Rh Typing Critical in Pregnancy?

Rh incompatibility — a critical concern in pregnancy:
  • If an Rh– mother carries an Rh+ baby (inherited from the father), small amounts of the baby's Rh+ blood can enter the mother's circulation during delivery, causing the mother to produce anti-Rh antibodies.
  • In a subsequent pregnancy these maternal antibodies can cross the placenta and attack the Rh+ red cells of the next baby, causing Haemolytic Disease of the Fetus and Newborn (HDFN) — a serious condition causing severe anaemia, jaundice and in severe cases hydrops fetalis or stillbirth.
  • Prevention — Rh– mothers are given anti-D immunoglobulin (RhoGAM injection) during pregnancy and after delivery to prevent sensitisation. This is why blood group testing in the first trimester is mandatory for all pregnant women.
  • Rh typing is also critical after miscarriage, ectopic pregnancy or any invasive obstetric procedure in Rh– women.
